Registration Checklist CHECKLIST OF ITEMS TO BRING WITH YOU TO REGISTRATION:

 

_____ 1. COPY OF A BIRTH CERTIFICATE OR PASSPORT:

    The Club keeps this copy. California Youth Soccer Association will not accept a hospital certificate. All players to Montclair cannot be registered without this.  Returning players may ask their last year's coaches for their copies back. We understand that this has been provided twenty million times before, however CYSA wants all of them just one more time.  CYSA is promising to lock down the information so that once and for all we don't have to keep asking for this.  NO COPIER WILL BE AVAILABLE AT REGISTRATION, PLEASE MAKE YOUR COPIES BEFOREHAND.

 

_____2. TWO SIGNED COPIES OF THE 1601 FORM. 

    This will be pre-filled in for you upon completion of your online registration and will serve as your confirmation of payment. We need 2 copies as one goes to your coach to keep on the field in the event of an emergency and the other hard copy goes to CYSA. There will be a very limited supply of 1601 forms at registration. Please make every possible effort to register online before coming to registration.

 

NO COPIER WILL BE AVAILABLE AT REGISTRATION DAYS, PLEASE MAKE YOUR COPIES BEFOREHAND.

 

_____3. 1" BY 1" PHOTO OF YOUR PLAYER (FOR U9-U19). ALL COACHES AND ASSISTANT COACHES MUST PROVIDE A PHOTO.

    U6 and U8 age groups do not need a photo. PLEASE TRIM YOUR PHOTO. It must be able to fit on a player pass the size of a drivers license. The photo must be full face, no profile. Write your child's name on the back of the photo and either use clear tape or a paperclip to attach it to your 1601 form .  No staples please.

NO HATS, NO BASEBALL CAPS, NO SUNGLASSES IN THE PHOTO.

 

_____4. COACH DISCLOSURE FORM 1628:

    This is only required for Coaches and Assistant Coaches. This will be pre-filled for you when you register online. You will receive a copy via email.  Please remember that coaches in U9-U19 need a photo too.  Please no hats or sunglasses.

 

_____5. JLYSSL Zero Tolerance Policy Form :

    All parents and coaches are required to sign this document in order to play on a Montclair Soccer team or within Jack London Soccer League. This is a new document for 2007. 

 

 

_____6. JLYSSL Coach Agreement Form:

    This is only required for Coaches and Assistant Coaches. This is a new document for 2007.

 

Please view the registration process for instructions on turn in dates for Montclair Soccer.  If you missed the turn in, please mail in your packet in the following order:

1.  The 2 copies 1601 forms that come after you have completed the online portion in Sportability.  Please don't forget to sign.

2.  The copy of the birth certificate and/or passport

3.  The zero tolerance forms

4.  Place the trimmed photo (U10-U19 groups) over the CYSA logo on the 1601 and use tape to secure..no staples please.  Don't forget to put the player name on the back.
